What evidence should an IVMS supplier give you?
Before a PDO or OQ tender, a contractor in Oman has to buy IVMS from a supplier the client will accept. The OPAL IVMS vendor registration form (the published PDF is named v1.1) lists what a supplier reports: the product details, the TRA certificate number and its validity, the commercial registration, the OPAL membership, CVC and HSEMS validity, and the HEMP and QA/QC submissions. This checklist turns those fields into evidence you can ask for, date, and file. The rows for an OPAL certificate, the last audit date and the official list are our additions, not fields of the form: ask OPAL what applies to you.
The sheet treats the two records differently. A TRA certificate belongs to a device model. The OPAL record is a separate paper for the supplier and the model: the form we read does not name an OPAL certificate, so ask OPAL what it issues and to whom. Check both at the source yourself.
How to use the checklist
- Settings. Enter the tender name and the check date. Use the bid date to test validity on that day.
- Supplier register. Add one column per supplier. Type values from the certificates, not from a sales brochure.
- Evidence check. Mark the 19 items. Use the calculated statuses in the register for the validity and audit items.
- Official lists. Look up each supplier and model. Record the result, the date, and who checked.
- Decision summary. Read the result, close the gaps with the supplier, and update the sheet.
How the score and the result are calculated
In the example, supplier A has 18 items Evidenced and 1 Partial: (18 + 0.5) / 19 = 97%. Supplier B has 5 Evidenced, 5 Partial, and 9 Missing: (5 + 2.5) / 19 = 39%. Two of its Critical items are Missing, so the result is Hold.
The minimum score (90%), the warning window (60 days), and the audit limit (6 months) are typical values. Change them in Settings to match your own rules.
Dates that need care
A certificate that is valid today may expire before the contract starts. Set the check date to the bid date, then to the start date, and read the status each time. The audit limit is your own rule. The form has year 1 and year 2 surveillance and re-evaluation entries in its OPAL-use block, so confirm the current audit rule with OPAL. If your client requires a certificate, confirm its status: an expired or unrecorded one shows a note only and does not change the result.
Limits
The checklist records what a supplier gave you. It does not approve a supplier or a device, and it does not replace any official list kept by OPAL or the regulator. The dates and numbers are only as good as the copies you hold.
