Installer QA/QC audit checklist with HEMP and JSA

Audit a technician on a real job, list the hazards before the work starts, and log every fix. An example procedure, 36 checks and a job safety form in one workbook.

Free: enter your name and email and we send you the file. Works in Excel, Microsoft 365, Google Sheets and LibreOffice Calc. No macros.
  • 36 checks in 5 sections
  • Risk score from 1 to 25
  • Corrective action log
Needs HSE review before use

This is a generic job safety form. It does not say that it meets any client standard. Your HSE lead should review the hazards, scales and controls before the first job.

What's inside

One workbook, five jobs

Write the procedure, plan the job, audit the technician, fix what you find, and read the result.

  1. 01

    QA-QC procedure

Eight clauses with example wording: scope, roles, incoming material, steps, tests, records, nonconformance, audits.

  2. 02

    Audit checklist

36 checks with Pass, Fail or N/A, a critical flag and a calculated status.

  3. 03

    HEMP and JSA

Steps, hazards, likelihood and severity before and after controls, with a risk matrix.

  4. 04

    Corrective actions

Source, finding, action, responsible, due date and a calculated overdue flag.

  5. 05

    Summary

Pass rate by section, critical fails, JSA check and the signature block.

The risk score

Score the hazard before and after the controls

Each line gets two scores. If the score after controls is still High, the sheet flags it.

  • Low
  • Medium
  • High
Likelihood by severity, example scores with the typical limits 5 and 12
Likelihood \ Severity1 Negligible3 Moderate5 Catastrophic
5 Almost certain5Medium15High25High
3 Possible3Low9Medium15High
1 Rare1Low3Low5Medium
Score = likelihood x severity Level = High from 12, Medium from 5, otherwise LowExample: 4 x 5 = 20, HighLikelihood and severity run from 1 to 5. The limits 5 and 12 are typical. Set the limits in your own HSE procedure.

Typical value. Replace it with the limits of your own HSE procedure.

The audit

Five sections, one real job

Walk the job with the technician. The critical flags are suggestions: change them to match your procedure.

What QA/QC means for an installer

QA is the written way you work: who does what, with which material, and what you record. QC is the check that the work on a vehicle really followed it. The procedure sheet covers the first. The audit checklist covers the second.

Device steps, fuse values and mounting positions come from the manual of the device you install. This file does not define them, so the procedure points to your installation sheet.

HEMP and JSA

HEMP (hazard and effects management process) and JSA (job safety analysis) both list the steps of a job, the hazard in each step and the controls. Some clients ask for one, some for the other. The sheet works for both. Ask your client which form it wants.

  1. List the steps. From the drive to the site to the road test.
  2. Score each hazard. Likelihood and severity, from 1 to 5.
  3. Write the controls. Then score the hazard again with the controls in place.
  4. Sign with the team. Everyone who does the job reads it first.

How to run an audit

Audit on a real job, not in the office. Stand with the technician, ask to see the tools and the vehicle wiring information, and check the work against each line. Mark N/A only for things that are not fitted. Log every Fail on the Corrective actions sheet with a person and a date.

Many companies audit each technician every quarter. Choose the interval that fits your risk and your contract.

Critical checks

A critical fail is one that can hurt a person or damage the vehicle: no battery isolation, a wrong fuse, a bare conductor, a cable near an airbag module, a new warning lamp after the job. One critical fail makes the audit result Failed, whatever the pass rate is.

Your client's forms

Source and date checked

Some oil and gas operators and programs ask IVMS suppliers and installers for a HEMP and a QA/QC program when they register. Read the current official text of your client, for example the registration form of OPAL in Oman, and use its own form if it has one. The OPAL IVMS vendor registration form (v1.1) asks the supplier to submit a HEMP for IVMS installation and a QA/QC program for devices and the installation procedure. We read the official PDF, last checked 2026-11-03. This pack does not say that any client accepts it, and nothing here states that a Tracom product holds an approval. opaloman.om

Installing Tracom ST100 or M1N? Use this audit next to the device manual, then confirm the first message on the SCMS portal.

  • Tamper-proof security. Test the tamper alert in section 3 of the audit.
  • Emergency panic alerts. Press the panic input and see it on the platform.
  • Secure access and immobilization. Test the cut-off safely with the vehicle stationary.
  • SCMS portal. Confirm the first message and the vehicle details in one place.
  • Over-the-air updates. Update device software without a workshop visit.
Questions

Installer QA/QC and HEMP/JSA questions

Preparing for a client review? Talk to Tracom.

What are HEMP and JSA?

Two ways to list the steps of a job, the hazard in each step and the controls. HEMP stands for hazard and effects management process. JSA stands for job safety analysis.

Does this pack meet my client's requirements?

Not necessarily. It is a generic starting point. Read your client's current official text, use its own form if it has one, and have your HSE lead review this one.

Who should do the audit?

The installation manager or a QA reviewer who is not the technician being audited. Both sign on the Summary sheet.

Can I use it for devices that are not Tracom?

Yes. The checks are generic. Fuse values, positions and test steps come from the manual of the device you install.

Can I change the checks and the risk limits?

Yes. Edit any text, change the critical flags and set your own Medium and High limits on the HEMP and JSA sheet.

What is Tracom's role here?

Tracom makes the devices. This pack is a neutral tool for any installer. It gives no approval, certificate or guarantee that a client will accept it.
